The following office forms have been made available to help expedite our patient check-in process.

Medical Record Release Authorization Form
If you were a patient of Drs. Greenhow or Loucks prior to February 1, 2006, and would like your previous medical records forwarded to PEAK Orthopedics, please print and fill out the above form, and fax to PEAK Orthopedics at 303.699.5486.

Surgical Assistance Form
This form reviews the process of utilizing a surgical assistant during surgery, and may be presented to you during your pre-surgical office visit if an assistant is required for your procedure.

Total Joint Replacement Information Sheet
This form provides information for patients undergoing total joint replacement. Information includes antibiotic
prophylaxis for other surgical and dental procedures, recommended follow-up visits, and air travel.

Pre-Operative Instruction Sheet
Form given to patients at their pre-operative visit with information regarding medications, medical equipment, and other general information.

Patient Check-In Form
This form is presented to patients at their first visit and includes demographic, insurance and other information.

New Patient Medical History Form
This form is presented to patients at their first visit and includes medical history.
